Executive Department of Indiana Adjutant Generals Office Indianapolis [June 24th] 18[91] This certifies that the official records (of which I am the lawful custodian on file in this office) show that [Albert A. Junkins] joined for duty and was enrolled as a [Private] of company ["A", 138th] Regiment Indiana Volunteers at [Peru Ind] on the [4th] day of [May] 18[64], by [Capt. Hoover] and that he was dully mustered into the Military Service of the United States at [Indianapolis Ind] on the [27th] day of [May] 18[64] for the term of [100 days] [editors note: word 'years' appears on the form but is crossed off] by [Lieut Mills]Mustering Officer [Discharged September 30th, 1864. Indianapolis, Ind.] The certificate is given as official evidence as enlistment, service, and [Honorable discharge] of [Albert A. Junkins] of Company ["A" 138th] Regiment Indiana Volunteers.
